1.) Full Payment: $300 per location is due with your application, payable by cash, check, *credit card or debit card.
2.) Monthly Payments: $25 per location per month due by the 21st of each month.
Terms: This membership agreement will be the only document provided regarding membership dues; invoices and statements will not be provided. Member acknowledges that Member is purchasing an annual membership and, in the event of Cancellation during the membership period, member agrees to pay the balance of the annual membership fee with credit / debit card on file with CCDRA. Dues paid in one payment: Cash*, check or credit / debit card accepted. Dues paid in monthly payment: Cash*, credit / debit card accepted. Credit / debit card payments are processed monthly on the 21st. Checks will be accepted when a credit / debit card has been provided and Member agrees for the credit / debit card to be charged in the event payment has not been received by the 21st of any month during the membership period. *Cashier’s check or money order will be considered a cash payment.
